Juergens Chiropractic is in-network for most insurance company PPO Plans, Medicare and Medicare Advantage PPO plans. Our staff can verify your insurance coverage. Benefits quoted to us by your insurance company are not a guarantee of payment.

We recommend that you contact your insurance company before your first appointment to verify your coverage, copays, and deductibles, referrals, or pre-authorizations that may be required for coverage.

We will gladly submit your insurance claims for you. lf your insurance company covers the quoted benefits, you may still be responsible for copays, deductibles, and any noncovered services.

Patient Forms

Please click on the link above to access our New Patient forms. Your can complete the forms on-line and submit them to our office, or print them and bring the forms to your appointment.

If you prefer to fill in the forms at our office, please arrive 15-20 minutes prior to your appointment time.

Consultation

At your first visit, you will have a consultation with the doctor to discuss your health-related problems, concerns, and potential/available treatment options. as well as potential and /or available treatment options.

History and Examination

In order to determine the cause of your actual problem, the doctor will ask you various questions related to your condition and your health history.

X-ray Studies

Your specific condition may require us to take some x-rays. We offer digital x-rays to reveal the internal structure and alignment of the spine.

X-rays may provide a blueprint for correcting the spine back to optimal health and alignment.

Same Day Treatment

Once finished with the examination and all appropriate diagnostic studies, patients will generally be provided with their first treatment during this visit. This treatment may include specific spinal adjustments, physical therapies, and/or soft tissue therapies.

Treatment Plan

At your next visit, you and the doctor will discuss your individualized written treatment plan and review the x-rays taken at the previous visit.

Various treatment options for your condition will be explained and the doctor will recommend a course of care. The “Three Phases of Care” will be explained, and the doctor will assist you in choosing any or all of the phases depending on the care you need.
